Insurance Information
We accept most major insurance plans.
Appointments
Please call the office to make an appointment in advance. All new patients are required to fill out medical forms and to arrive 15 minutes early so that we may review it with you. If you are unable to keep your appointment, please call far in advance to cancel or reschedule as other patients certainly will appreciate the opportunity to use that time. We make a sincere effort to adhere to our appointment schedule and appreciate your patience when the office is behind schedule due to emergencies or hospital surgery.
Fees & Payments
We make every effort to keep your healthcare costs to a minimum. Therefore, we require payment including all co-pays for services rendered at the time of your visit unless other arrangements have been made. We accept cash, checks, MasterCard, Visa, and Discover for your convenience. If we are a participating provider with your insurance company, we will bill them as a courtesy; however, all charges are the patient's responsibility. Please let us know if your insurance changes immediately and if we can to assist you in handling your claims. There is a $30 charge for all missed appointments.
Prescriptions & Renewals
Please request all prescriptions and authorizations for renewals during our regular
office hours, when our full records are available. Renewal requests are not filled after hours, weekends or when the office is closed.

YNN.com, for up-to-date weather closings. Our office staff will attempt to contact our patients in the event of a closing or delay when possible.
